Information for patients of closed and temporarily closed practices
If your family doctor has closed their practice you must join the Provincial Patient Registry to get on a waitlist for a new family doctor or nurse practitioner (unless stated otherwise below).
You are also eligible to register with Maple, Health PEI's virtual care provider for people without a family doctor or nurse practitioner. If your former doctor is listed below, you can register before you join the Provincial Patient Registry. If your former doctor is not listed below, you must join the Provincial Patient Registry before you register.

Information for patients of Dr. Chingling Yoong
All former patients should join the Provincial Patient Registry, a provincial waitlist for a family doctor. If you were a patient of Dr. Yoong for less than one year, when you rejoin the Provincial Patient Registry your name will be assigned at the date you first joined the registry.
Information for patients of Dr. Harold Molyneaux
Your health records are now in the care of Health PEI. A process has been started to upload your relevant health information into the Provincial Electronic Medical Record system. This will take some time. Until December 1, 2024, you can request copies of your health records by calling 902-374-1631 and leaving a message with your:
- name
- PEI Health Card number (provincial health number)
- date of birth and
- telephone number
A staff person will call you back within seven business days to confirm what information you are requesting and provide you with further direction.

Information for patients of temporarily closed practices
Harbourside Health Centre
- Patients who receive a letter from the centre will temporarily be without an in-person primary care provider for between one and four months from the time they receive the letter. You don't need to join the Provincial Patient Registry as you will be assigned a new provider.
Dr. Syed Naqvi
- Note: Dr. Naqvi is still considered your primary care provider while he is on leave. You don't need to join the Provincial Patient Registry.
