Municipal Election Documents

The Municipal Government Act and its regulations require that certain information be recorded or published in a particular form.  There are forms that must be filled out by candidates, election officials, the Chief Administrative Officer and electors/voters under certain circumstances. These forms are all listed below.  

 

Election Forms

Election Official Declaration Form Elections officials must make a declaration that they will faithfully and impartially perform their duties in the administration of the election.  The Municipal Electoral Officer and Deputy Electoral Officer are appointed by council resolution so no form is required. Notice of Nomination The Notice of Nomination (which can be modified for a By-Election) must be posted in at least one print format (e.g. posters, newspaper, or newsletter) and one electronic format (e.g. website, Facebook, e-newsletter).  The notice must be posted at the election office and the municipal office.  Notice of Nomination for an Election

Oath of Elector Form is used at an election for a variety circumstances (e.g., proof of residence, eligibility is challenged, when an elector requires assistance voting, discrepancy with official list of electors, r name struck out in error). Oath of Elector
Candidate Nomination Form Used by a candidate to obtain nominations in order to become an official candidate for Mayor or Councillor. Candidate Nomination Form
Candidate Withdrawal Form Used by a candidate to withdraw from nomination and election proceedings.  Candidate withdrawal Form
Agent Form Authorizes a candidate to appoint an agent to represent them.  Agent Form
Campaign Contributions and Expenses  All candidates (whether elected or not) must file a Campaign Contributions and Election Expenses Form within two months of the election.  Campaign Contributions Election Expenses form
Disclosure Statement All council members must file a Council Member Disclosure Statement with the Chief Administrative Officer (CAO) of the municipality.  Disclosure Statement
Council Oath of Office All council members must swear or affirm an oath of office.  Council Oath of Office
Election Summary Report

The CAO must the Election Summary Report to Municipal Affairs within 10 days of being notified by the Municipal Electoral Officer of the elected candidates.

Municipal Affairs has a number of templates that can assist with running an election.

Election Tally Sheet This sheet is used by the Poll Clerk and at least two others to mark the vote count. Download Election Tally Sheet
Election Recap Sheet (At Large) The Recap Sheet is completed by the Returning Officer after they receive the election materials from the Deputy Returning Officer.  The Municipal Electoral Officer will verify the Recap Sheet.  This sheet is for an 'at-large' system. Download Recap Sheet (At Large)
