You may use this online form to request access to government information. You will need to be specific about what records you want to access and which department has these records. It will help narrow the search if you can clarify the period of time related to the information.

How do I request access to my personal health information?

To request access to or a copy of your personal health information, contact the records department of the health facility where you received care (for example, at your primary care provider’s office or at the Health Records Department of the hospital where you were admitted).

Is there a fee for requesting information?

There is an initial fee of $5.00 and there may be additional costs for requests that require greater than three hours to process.  If photocopying is required, the cost is eight cents per page.

How long will it take to process my request?

It may take up to 30 days to respond to a request for information.
