Home heat tank means an above ground petroleum storage tank that has a capacity of 2200 litres or less, is installed in a fixed location and is connected or intended to be connected to oil burning equipment. Only a licensed installer can install, alter, or remove a home heat tank.

This form also includes a second page with instructions on how to fill it out when more than one contractor requires payment. Please follow these instructions carefully when sending in the Request for Payment Form.

Applicants must sign and return the Request for Payment Form and all invoices before payment will be made.
