You can request a replacement for a lost or damaged Health PEI Card at any Access PEI location. 

If you are unable to visit an Access PEI location, mail a completed PEI Health Card Replacement Request Form with a cheque or money order to the address on the form.  

There is a $10 replacement fee for each card, with a maximum of $50 per household.

This form is used to apply to the Independent Study (ISC521A or ISC621A) program in senior high school.

This form is to be completed by a referrer to the Strength Program.
