This form is required to be completed by a Physician as part of the Change of Gender application process.

At the time of registering a deed of conveyance all purchasers must attached a completed Affidavit of Purchaser form and forward the documents to the Registry Office.

The purpose of the Grant-In-Lieu Taxes program is to provide limited financial support for approved non-profit organizations, community groups, or municipalities that are providing specified services in the community by forgiving (crediting) non-commercial provincial property taxes.

Complete this form when adding, changing or deleting the name or address of the designated taxpayer.
