Health records contain information such as reports from care providers, results of tests such as blood work and diagnostic imaging (X-ray, MRI, etc.), and prescribed medications. Health records can be in paper or electronic format, or a combination of both. Your personal health information is confidential and protected against loss, theft, and unauthorized access and will be kept for as long as required by law.
