How do I request access to my personal health information?
To request access to or a copy of your personal health information, contact the records department of the health facility where you received care (for example, at your primary care provider’s office or at the Health Records Department of the hospital where you were admitted).
Is there a fee for requesting information?
There is no fee to request access to personal information from government. However, if photocopying is required, the cost is eight cents per page.
Is there anything else I need to know?
- You must provide proof of your identity before records are released to you.
- If you are requesting records on behalf of someone else, you must provide proof that you have legal authority to act for the person (ie, guardianship or trusteeship order, power of attorney).
How long will it take to process my request?
It may take up to 30 days to respond to a request for information.
