Patient Navigator

The health service system is complex and difficult to understand, particularly for those who are suffering, in pain and are vulnerable. The patient navigator service supports individuals and caregivers, helping to connect them with the right health care provider. The patient navigator will work to identify and reduce barriers that prevent patient’s timely access to health services.

The patient navigator will:

  • liaise with health care providers and programs who can support you;
  • help you find and access community resources;
  • act as a single focus point for coordination of health care services you require; and
  • answer your questions about the health care system.

Health PEI is committed to improving the flow and care of patients both in and out of hospital.
