Municipal Election Documents

The Municipal Government Act and its regulations require that certain information is recorded on a “form approved by the Minister” for municipal elections.  There are forms that must be filled out by candidates, election officials, the Chief Administrative Officer and electors/voters under certain circumstances.  

Forms for Election Officials

The Municipal Electoral Officer (MEO) and Deputy Electoral Officer (DMEO) are appointed by council resolution, no form is required.

Election Official Declarations

A Returning Officer must make a declaration in front of the Municipal Electoral Officer (MEO) that they will faithfully and impartially perform their duties in the administration of the election.

Any election official that a Returning Officer (RO) appoints (Deputy Returning Officer, Election Clerk, Poll Clerk, or Information Officer) must make a declaration in front of the RO that they will faithfully and impartially perform their duties in the administration of the election.

Notice of Nominations

The Municipal Elector Officer must publish the Notice of Nomination for an Election which can be modified for a By-Election in at least one print format e.g. posters, newspaper, or newsletter and one electronic format e.g. website, Facebook, e-newsletter. Additionally, this notice must be posted at the election office and the municipal office.

The Municipal Elector Officer must publish the Notice of Election which can be modified for a By-Election in at least one print format e.g. posters, newspaper, or newsletter and one electronic format e.g. website, Facebook, e-newsletter. Additionally, this notice must be posted at the election office and the municipal office. 

The notice must contain information on:

  • wards for which an election shall be held, if applicable;
  • the location and hours of the advance poll;
  • the location and hours of the polling stations on election day; and
  • the type and number of identification documents that are required  of electors/voters at the voting stations.

Election Forms Related to Voting

  • Currently, only Summerside, Charlottetown, Cornwall, Three Rivers and Stratford allow Mail-in Ballots.  Application for a Mail-In Ballot
  • Oath of Elector form is required for the following circumstances:
    • if an elector does not have the required proof of residence when they are told at the polling station
    • if an elector's name is not on the official list of electors or their eligibility to vote is challenged
    • if to vote an elector requires assistance voting
    • if there is an error on the official list or electors
    • if an elector's name is struck out in error from the official list of electors and the elector has not yet voted.

Form for Candidates 

To become an official candidate for Mayor or Councillor complete the Candidate Nomination Form.  

A candidate can withdraw a completed nomination form by submitting a Candidate withdrawal Form. If the form is withdrawn before 2:00pm on nomination day, then the candidate is considered to not have been officially nominated.

Candidates can appoint agents by completing an Agent Form.

All candidates, whether they are elected or not, must file a Campaign Contributions Election Expenses form with CAO within two months of the municipal election. .

All elected council members must file a Disclosure Statement with Chief Administrative Officer of the municipality.  After elected every council member must complete an Council Oath of Office.

Election Summary Report

Within 10 days of being notified by the Municipal Electoral Officer of the elected candidates, the CAO must submit an Election Summary Report to the Minister.

Election Recap Sheet (At Large) The Recap Sheet is completed by the Returning Officer after they receive the election materials from the Deputy Returning Officer.  The Municipal Electoral Officer will verify the Recap Sheet.  This sheet is for an 'at-large' system. Download Recap Sheet (At Large)
